Dr. Brad Sullivan, who leads our women’s pelvic health services, is a minimally invasive gynecologic surgeon specializing in endometriosis, chronic pelvic pain, and infertility. He brings this surgical expertise to every appointment, offering additional context that other specialists may not provide, such as assessing anatomical factors that may affect surgical risk and providing insight into potential surgical approaches. Dr. Sullivan also offers consultation services, guiding you while ongoing treatment and follow-up remain coordinated with your referring physician.

Whether you need a routine look at your reproductive organs or specialized imaging for endometriosis, infertility, or other pelvic health concerns, our team offers a range of women's health ultrasound services.

Basic Pelvic Ultrasounds examine the uterus, ovaries, and fallopian tubes to assess the health and structure of these reproductive organs and to identify potential abnormalities, such as adenomyosis, fibroids, PMOS/PCOS, or ovarian cysts and masses. Our expert ultrasounds are based on cutting-edge research to give you and your provider the best information on which to make decisions about your health.

Advanced Pelvic Ultrasounds will examine the uterus, ovaries, and fallopian tubes, as well as common sites of endometriosis, such as the rectum, bowel, or pelvic ligaments. We evaluate for superficial endometriosis and can diagnose early-stage endometriosis, which cannot be seen on MRI or routine ultrasounds.
Our team also offers specialized ultrasound procedures to provide additional information about the uterus, fallopian tubes, and pelvic structures:

Sonohysterogram: Uses sterile saline to improve visualization of the uterine cavity and identify abnormalities such as polyps, fibroids, or scar tissue.

What to Expect During a Pelvic Ultrasound at Milestones Diagnostics™
1.
Prepare for your appointment beforehand
Your preparation will depend on the type of ultrasound or procedure you're having. For most pelvic ultrasounds, you don't need a full bladder. For an advanced endometriosis ultrasound, try to have a bowel movement beforehand and eat a light meal. You can continue your usual medications, including endometriosis medications, unless your physician tells you otherwise.

Check in and get comfortable
When you arrive at Milestones, you'll check in with our reception team and be asked to empty your bladder and change into a gown. Your ultrasound will typically be performed transvaginally or, alternatively, transrectally, depending on your individual needs and the type of examination.

Your sonographer or physician performs the ultrasound
Your ultrasound will be performed by a sonographer or male physician on our team, typically taking 30–45 minutes for a basic pelvic ultrasound or 45–60 minutes for advanced endometriosis imaging. Your specialist will examine your pelvic organs and, when indicated, assess areas where endometriosis may occur. Advanced ultrasounds for endometriosis differ from a basic ultrasound, as the probe may be gently repositioned in and out of position to assess how your pelvic organs move and identify scar tissue or adhesions. It’s important to know this if you’ve had negative experiences with ultrasounds in the past. Tell us if you experience pain with vaginal penetration or examinations.

Additional procedures may be performed
If you're having a sonohysterogram, tubal patency examination, or SonoPODography, sterile saline may be introduced through a thin catheter to improve the accuracy of a pelvic ultrasound. This sterile saline is instilled into the uterus through a very thin catheter, which is passed through the cervix and into the uterus via a speculum exam. Some patients find these procedures uncomfortable and can take an anti-inflammatory drug to ease pain. Your specialist will explain what they're doing throughout the procedure and answer any questions you have.
5.
Discuss your results and next steps
After your examination, your specialist can discuss the findings with you and, when appropriate, provide additional context to your referring physician. If you're experiencing chronic pelvic pain or infertility, consultation services may also be available to help guide your ongoing care.
